Microsoft Adaptive Controller
Xbox and PC gaming. Pairs with Xbox One controllers to allow for remapping buttons and commands. Base kit has 2 large buttons, along with a small d-pad. The top of the device has 19 3.5mm jacks and 2 USB ports that correspond to each button or trigger on a Xbox One controller. You can purchase accessories for these ports, anything from arcade-style buttons, to joysticks, to foot pedals. There is a learning curve to setting it up, but there is good existing documentation and customer service. For me, it allows me to re-assign the left trigger and bumper to my feet, and can switch repetitive button presses around when my hand is in pain. Could possibly be used for other computer needs, but I haven't looked into that. Side note, the packaging it comes in is incredibly well designed for easy opening!
